North Texas Behavioral Health Authority (NTBHA), based in Dallas is responsible for developing a collaborative system of care for children, adolescents and adults with severe mental illness and other serious emotional disturbances, as well as those experiencing a substance use disorder. This position’s actions and work must align with NTBHA’s values and goals, policies and procedures, and meet all documentation standards and regulatory requirements.

The Community Health Worker is responsible for assisting in delivery of services that address the social determinants of health, and bridging gap between communities, individuals and providers. Conducts community outreach and education.
Requirements:

Must have Texas Department of State Health Services (DSHS) CHW certification or meet competencies to become one within 6 months.
May be licensed or certified as a Recovery Coach, Peer Specialist, Licensed Chemical Dependency Counselor (LCDC), licensed Nurse or Social Worker.
Preferred minimum of one (1) year of experience in one or more of the following:

o Substance use outreacho Substance use interventiono Substance use treatment
Preferred minimum of one (1) year of experience in at least two (2) of the following:

Working with prison populations
Working with individuals experiencing housing instability
Working with individuals with Substance Use Disorders (SUD), HIV/STDs, and/or behavioral health issues
Community health work
Ability to maintain and operate company vehicle

Job Duties:

Spend, at minimum, ninety-five percent (95%) of work time delivering direct participant services in the field.
The position calls for extensive travel. You should expect to be on the road traveling to and from locations when not engaging in the community. 5% to 20% or more could be required travel time.
Increase linkage and retention in substance use, mental health, and medical services for Texas residents living with substance use disorders (SUD).
Address behavioral health disparities in the program service area.
Increase opportunities for substance users, including opioid users, to reduce harms related to substance use.
Help individuals address medical needs.
Help individuals who desire change to build a foundation for their recovery.
Keep track of services being provided.
Complete task as needed to fulfill job responsibilities.
Create, analyze, prepare, and maintain records, files, documents, reports, and correspondence.
Identify and facilitate solutions for recognized problems.
Use effective written and oral communication skills with individuals from diverse cultural and socio-economic backgrounds.
Constructively and efficiently work with other professionals and staff, community representatives, persons with disabilities, and family members.
Develop effective inter- and intra-team relationships to encourage and support communication among various organizations.
Identify and assist vulnerable individuals and/or individuals with complex and unmet SUD needs in underserved populations.
Provide referrals to OSAR, NTBHA SUD network providers, NTBHA MH network providers, medical and community resources.
Assist with patient access to community and governmental service agencies.
Educate and encourage individuals and communities to adopt healthy behaviors; provide information on available resources; promote preventative care and screenings, etc.
Engage in ongoing education and professional growth, with the guidance and in collaboration with the supervisor, by attending in-service sessions, department meetings, workshops, and reading journals to keep abreast of current trends in the field. Integrate knowledge gained into current work practice, with support of supervisor.
