JOB PURPOSE:
The Case Manager is responsible for assessing the needs of unaccompanied children in care, developing Individual Service Plans, screening for human trafficking concerns, facilitating the safe and timely release or discharge of children and youth, and documenting the provision of services in the case files. The Case Manager will work primarily to help unify unaccompanied children who are residing at Endeavors Pecos Children’s Center (PCC) in Pecos, Texas.
ESSENTIAL JOB RESPONSIBILITIES:
Case Management services, as described below:
Oversees all aspects of the Unaccompanied Child’s (UC) case and provides release recommendations for a safe and timely release.
Maintain case file documentation in accordance with ORR requirements.
Maintain confidentiality when gathering, documenting, consulting, or consolidating information necessary for the case management assessment and the plan of care.
Maintain regular contact with each UC, UC’s family when possible, and established service providers to communicate case planning and progress.
Provides client services as needed, including but not limited to making medical appointments, finding resources, etc…
Gather historical and current data relative to completion of a comprehensive case management assessment including, but not limited to, school records, medical records, mental health treatment, legal status and court involvement, social and family strengths and needs, support systems, and specific cultural issues.
Presents case to federal stakeholders as requested
Participate in team meetings, staff in-service training, and department meetings.
Carries a caseload of up to 12 clients, varying on program census.
Completes Signification Incident Reports (SIRs), per federal policy requirements.
Conducts internal chart reviews to monitor documentation compliance.
Completes, timely and accurate timesheets and other personnel/administrative records as required.
Other duties as assigned.
